Verification Active Diplomates
The American Board of Dental Public Health (ABDPH) requires a $25 fee per verification payable to the ABDPH. The request should be submitted in writing, and must include a copy of the signed general release of information by the dentist who's Board status is being verified.
The verification request and payment should be mailed to:
